DaVita is seeking a part time Patient Pathways Liaison to facilitate the seamless transition of patients from Mosaic Life Care in St. Joseph, MO and St. Luke’s North in North Kansas City, MO hospital settings to our outpatient dialysis care. This role is ideal for a coordinator or clinical professional who excels at insurance verification, hospital collaboration, and patient advocacy. You’ll empower patients facing kidney disease by providing vital education and ensuring they have a seamless transition to the dialysis center of their choice.

Location:

  • Locations: Mosaic Life Care in St. Joseph, MO and St. Luke’s North in North Kansas City, MO

  • Perks: No Weekends. No Holidays. No On-Call.

Schedule:

  • Part-Time: Not to exceed 24 hours per week.

  • Shift: 5 variable hours per day, Monday–Friday (scheduled between 8:00 AM – 5:00 PM).

What You Bring

  • Experience: Minimum of 2 years of recent healthcare experience is preferred.

  • Specialized Knowledge: Dialysis experience is a huge plus and highly preferred!

  • Education: Vocational Certificate or Associate’s Degree.

  • Skills: Proficiency in insurance verification, navigating healthcare systems, and high-volume coordination.

  • Mindset: A passion for clinical education and a desire to help patients make informed healthcare decisions.

What You’ll Do

  • Educate & Advocate: Meet with patients in St. Joseph and North Kansas City to discuss dialysis options, insurance, and treatment modalities with empathy and clarity.

  • Facilitate Placements: Coordinate with hospital teams to manage referrals and ensure all medical documentation is ready for a smooth transition.

  • Build Partnerships: Strengthen relationships with local hospitals and doctors to improve the patient experience across the Missouri region.
