Division Lead Social Worker

The Division Lead Social Worker will oversee approximately 2 divisions, ensuring the success of all social workers within these divisions. This includes coaching regional leads, preceptors, and social work teammates. They possess expanded social work knowledge, help develop and deliver social work policy and initiatives, and educate social work peers. The division lead will be responsible for executing the social work strategy set by the Group Leads and Clinical Transformation, reinforcing DaVita P&P and external regulations. Meets standards for Social Work and performs essential duties & responsibilities outlined below. Some, but not all, DLSWs will have a patient caseload and/or other assigned divisional duties. The Divisional Leads report to the Group Lead Sr. Manager.

Essential Functions:

  • Conducts ops review with local leadership and reports out to the Group Lead for outlier management

  • Is the division SME for social work-related survey readiness, social work training/requirements, etc

  • Oversees social work pilots, initiatives, and P&P rollout within the division

  • Support Regional Leads and Preceptors in driving the recruiting, training, and onboarding of new social work TMs within the division.

  • Identifies areas of continuous improvement and drives the development of social work TMs.

  • Contributes significant expertise beyond the regional level to the divisional and/or Palmer and potentially national DaVita Social Work team (e.g, Village Subject Matter Expert)

  • May fulfill the duties of a clinic social work teammate for a set amount of hours a week, determined by the patient census.

  • Other duties as assigned.

Education and Experience:

  • Master’s Degree in Social Work from a school accredited by the Council on Social Work Education Required

  • MSW with 24 months of specialized renal experience Required

  • Schedule flexibility and ability to travel Required

  • Intermediate computer skills; Proficiency with Microsoft Office (i.e., Word, Excel, and Outlook) and DaVita applications Required

  • State licensure or certification is required if licensure or certification is available in the state where the facility is located. Required

  • LCSW or equivalent license, even if not required by the state Preferred

  • Current driver’s license in the state of residence and an insurable driving record Preferred

  • 10% travel Required
