Educator, Patient/Clinical Services
Kidney Smart Educators perform an important role in giving CKD participants the gift of education! Kidney Smart is a patient education program covering CKD education at no-cost to help empower patients to make more informed choices – no matter where a patient may eventually start dialysis and regardless of their provider, payor or employment status.
We are looking to hire a Full-Time (40-hours a week) resource who would work on a Monday-Friday (typically 8am-5pm or 7am-4pm) flexible schedule. This role travels to DaVita Dialysis clinics within the Region including by not limited to: Yonkers, Tarrytown, White Plains, Carmel, Port Chester, Stamford, Bridgeport, Danbury, and Torrington. 20% remote option when not training/educating out in the field!
Role Description:
Kidney Smart Educators should be friendly, warm, and welcoming in nature with a passion for CKD education. Kidney Smart Educators are ambassadors who promote the program when opportunities arise. Kidney Smart Educators must be clinically licensed or certified healthcare professionals and DaVita teammates, In order to become a KS educator, teammates must present proof of certification or clinical licensure upon onboarding as a KS educator. This can be confirmed by presentation of a hard copy upon onboarding or through verbal confirmation from direct manager who hired teammate. Note: Any Kidney Smart Educator that was not licensed or certified prior to May 18, 2016 may remain a certified Kidney Smart Educator. However, if the Educator is removed from the KS roster at any time he or she must complete all onboarding requirements to be added back as an “active educator.”
Responsibilities:
Education Delivery:
Engage the community with focus on key actions toward improved health outcomes for CKD participants through a classroom environment • Explain complex health concepts for participants in an easy-to-understand manner, using the most recently approved versions of the Kidney Smart curriculum(curriculum and educator guide)
Skilled in working with a wide variety of participants who maybe challenged in hearing, sight, and mobility
Keep the Kidney Smart Program safe by following all Kidney Smart Program Requirements as indicated in the compliance policy and Educator Resource Manual.
Skilled in reviewing, educating and answering questions on all modality options for dialysis including, transplant, hemodialysis, PD and HHD.
Administrative:
Complete all training courses that are related to Kidney Smart Compliance and Operations
Use only most recent approved Kidney Smart materials(curriculum, support materials and educator guide)
Plan class locations considering convenience and accessibility for area participants and technological resources available(if applicable)
Plan class dates and times to maximize efficiency and reach(if applicable)
Schedule classes through Kidney Smart scheduling system (if applicable)
Ability to use laptop/projector • Hold classes in approved, compliant venues per policy and procedures
Submit all class attendance forms within48 hours of each class completion to KS Local Coordinator
Call participants to remind them of scheduled classes & provide post-class follow-up(if applicable)
Commit to staying up to date with Kidney Smart communications and implementing all process/program changes
Read the Newsflash, attend WebEx meetings, and KS calls as scheduled in your local area
Outreach and Program Promotion:(If applicable)
Assist regional and divisional leadership in physician outreach, which may consist of, but is not limited to:
Presentations of class content, clinical impact metrics, and recommendation processes to nephrology practices
Developing
Educational and Clinical Requirements:
Clinical experience (kidney care preferred)
Educators must be certified and/or licensed healthcare professionals and must be able to present proof of license or certification upon onboarding as an educator.
